We faced the same problem (for journal articles with multiple authors with different affiliations), but did not find any good solution to it. One crude solution I was thinking about was to add numbered metadata fields, one for each author, e.g:
dc.contributor.author1 = Smith, John dc.contributor.author2 = Doe, Joe dc.contributor.affiliation1 = Smithsonian dc.contributor.affiliation2 = Doe's University That could work decently within dspace, but it would be quite a headache to have this kind of metadata exported over OAI in a sensible way.. So, essentially, we ignored the requirement to record this information for now..
