A DSpace fresh-install does not make thumbnails from pdfs.
The primary bitstream setting I believe is used when you submit an
html item with related files. In your case I don't see why you should
set a particular bitstream to be primary.
There is a patch from scott yeadon
http://sts.anu.edu.au/drs/downloads/index.php.
you will need to download and install ImageMagick, a great command
line soft for graphics.

I also have experienced the same problem in one of my item,
my item have bitstream like this :
File :
PDF (as primary bitstream)
PDF
PDF
PDF
JPEG
Then, I make a change, I set JPEG file as primary bitstream, then the
preview is available.

JPEG (as primary bitstream)
hope this work...if not...I don't know how to solve your problem...hope
