I've been working on making a Book Reader / Page Turner. It uses Open
Library's Book Reader software.

It currently works with having multiple image bitstreams, we don't have
auto-generating images from PDF's yet.
Lastly, have you considered using the XPDF media filter plugin, and making
large image thumbnails of each page of your PDF offline / at filter-media
time, and then just serving your PDF-THUMBNAILS bundle for display?
Also, if you want to just have Google do the streaming for you, its very
easy, unless you're a lawyer.
