I don't know anything about the solution described by Keiji, but I wrote a curation task earlier this year that adds cover pages to pdf files. While Keiji's code adds the coverpage on the fly every time the file is requested, my curation task actually replaces the PDF file with a version that includes the coverpage (the original file is moved to a different bundle in case it is ever needed again). It depends on your needs which of these approaches is better suited. You can find my code here: https://github.com/lconz-irr/Curation-Tasks -- it isn't documented or tested particularly well yet because "my" repositories are still on DSpace 1.6 and we're not using this task yet.
