Actually, DSpace already contains the checksum for each bitstream in the 'checksum' column of the 'bitstream' table. So it should be easy to add this check - after the new file is uploaded, calculate its checksum and if there's already such checksum refuse to accept it. This would probably be a welcome functionality to include in DSpace, so if you implement it, be sure to submit a patch.
