Hi Peter, I was haunted by this same bug almost every time when OAI was under load and in some other rare cases, too. But I gave up on solving it in favor of XOAI which doesn't have this problem because it uses Solr instead of database (it's configurable to use database if you want, but I haven't encountered the problem, but then I didn't stress test it, either).
