One observation, though: bitstreamaccessFilter *is* enabled for the driver context, so I don't know what went wrong there. https://github.com/DSpace/DSpace/blob/dspace-3_x/dspace/config/crosswalks/oai/xoai.xml#L39 And it isn't enabled for request context because it shouldn't be, by default.
