I can't speak to the 'related items' issue (although I think SOLR/Discovery may provide something akin to that functionality), but I can report that here at the University of California, Irvine Libraries, I am using the Disqus commenting plugin (https://disqus.com/), embedded at the item level, to allow users to submit comments for select collections on our DSpace implementation at http://ucispace.lib.uci.edu (e.g., see http://ucispace.lib.uci.edu/handle/10575/9839 at bottom of page).
