What DSpace version are you using? We made quite a big change to the citation_pdf_url behaviour in DSpace 4. You can see the full discussion around the change here: https://jira.duraspace.org/browse/DS-1483 but the expected behaviour in DSpace 4 and 5 is:

The "citation_pdf_url" is now provided as follows:

  • if the item has only one bitstream in the ORIGINAL bundle, link to this one
  • if the item has a primary bitstream in the ORIGINAL bundle, link to this one (regardless of how many bitstreams there are in total in the ORIGINAL bundle).
  • Bitstream must be publicly visible to be linked to in the "citation_pdf_url". The MIME Type of the bitstream is no longer checked, as Google Scholar has reported this is no longer important (see description of this ticket).

For items with multiple bitstreams but no primary bitstream, I believe this will select the first one according to the bitstream order. If you're on DSpace 4 or 5 and this behaviour isn't what you're seeing, please check that the bitstream is actually readable by Anonymous.
