I have never used anything except the header signature mode; I don't
have to deal with id10t's using lookOut being unable to forward messages
with headers intact. For one thing, I'm corporate, so I can require the
use of a real mail client like Thunderbird (being a BOFH has its
benefits). For another thing, my users just move the message into their
Junk folder and I pick it up nightly with a script. I don't even have
the -spam -nonspam aliases configured; life is much too short to fight
with half-measures.

I agree with John. We started with signatures in the message a while
back. The message re-writing process causes a tremendous number of problems.
We have the "save to a SPAM folder/harvest nightly" setup and when users
have problems we point them to the header tagging only. If we could eliminate
all message body re-writing, everything would work much better. As discussed
previously, signatures in the body of the message are extremely difficult to
get right and they mess up digital signing to boot.
OTOH, There is no other way to do it ASAIK if you cannot use IMAP.
