For Debian, it is managed automatically via dbconfig-common.

For example, changes for the upgrade from 3.8.0 to 3.9.0 for MySQL are
stored in the Debian sources as debian/sqlfiles/upgrade/mysql/3.8/ (as
per the UPGRADING file), and these SQL statements are automatically made
in postinst.

I have tested this for MySQL only, and not as much as I wanted.
It will be tested anyway before an upload to the official mirrors once
DSPAM 3.9.0 is released.
