Just getting in on this one late, but I'll add a "me too" to this one. 
Same amp, same HT.  Haven't seen any problems with lost headers and as a
Gateway admin, I have access to the D-Plus logs to REALLY check.

The amp does typically want just a tiny bit more power than my IC-91AD
will give on low power to trigger its RF switching circuit... but it's
right on the hairy edge.  Some combinations of this amp and the IC-91's
low power settings MIGHT work, but typically you'll have to use the full
5W... which will bake your IC-91AD in short order.  

(As others have joked, Icom should have sold the IC-91AD with a nice
Icom-branded oven mitt for full power use for more than a few minutes. 
It'll fry your hand.)

I would say with the four power level settings, the IC-92AD is a better
"match" if you don't need the full 30-35W output from this amp.  And as
someone else mentioned, if your mission is "mobile" operations 80%+ of
the time, buy a mobile rig and live without the HT instead of abusing an
HT and having to buy an amp anyway...

I've only tried this setup because we have a family truck that's
"shared" amongst two households for when a truck type vehicle is needed.
 Since there's hams in both houses, the truck has the Mirage amp
permanently installed, and a good quality SMA-to-UHF cable for hooking
up one's HT and a permanently mounted dual-band VHF/UHF on the roof
along with another antenna which is for extended listening, since it
also has a cheap garage sale scanner and yeah... there's even an old
Cobra CB in there with WX channels.  Not bad for the "junkbox" radio
installation done out of spare junk lying around so the truck would have
something in it.
