"Alternate Ethernet" is a device name that used to appear in the bad 
old days, but not what you should be using for your PC card. It 
refers to a different driver architecture which probably won't work 
at all under more recent versions of Open Transport (such as those 
bundled with OS 8.5-9.0). I suspect you have a software problem, 
nothing more.

The only extensions you actually need are the GV Platinum Pro Enabler 
(enables the dual-function PC card) and GV PC Card Ethernet (the 
actual ethernet driver for the device in these cards). The GV 56k 
Combo Enabler file(s) are equivalent to the Platinum Pro Enabler for 
people who have that card instead (the ethernet driver is the same). 
The ethernet driver I currently have is 3.0.3, which I believe came 
with the last update (the one required for OS 9), but which may be 
required for 8.6. You can certainly lose the Platinum Enabler (which 
isn't being used) and the Multifunction Update (which is rolled into 
the system).

As a side note, you don't actually need any extensions at all for the 
modem half of the card (or for any modem card), though you do if you 
want to use the GV software for sending or receiving faxes.

You don't need this file unless you're using a Mac with a bundled 
Apple ethernet device (which the 2400 doesn't have). It won't cause 
any trouble beyond slowing your boot time by a small fraction, but it 
does no good either.

Because the OS installer script isn't very smart, and no one bothered 
to tell it the difference between your 2400 and a 3400. It can go; 
same comments as above.

There was never an Alternate Ethernet extension - rather, it was a 
generic name for ethernet drivers that presented through a 
now-outmoded OS interface. The new driver interface (which is what 
"ndrv" stands for) will give each device its own name in the control 
panel pop-up. This all has to do with the switch from "classic" 
networking to Open Transport, in case you're wondering.
