Seem to remember hashing this out with you last time I tried to make MacAlly USB work on mine. Same set up, 2400 cardbus enabled with 8.6...

As far as I can remember we were on about Firewire PC cards for a while, but we only touched the USB stuff somewhat. I may be wrong though.

I ended up settling back on the earlier version of USB software (1.3.5)...

As I said several times, I am lucky enough to have a maxed-out 2400c that gets several hours of daily usage, runs like a champion at G3 speeds and crashes once every couple of months if ever. In my experience it doesn't make much difference whether you use the 1.3.5 or 1.4.1 version of the USB software with the MacAlly card. The trick is using the custom MCE modified drivers. I have used a good deal of USB scanners and digital cameras and stuff, never a problem.

Seem to have memory of an Apple PC card extension, which I don't seem to have in the extension folder - may be the reason.
None of the sort in my extension folder. The closest match is "AirPort PC Card", which you only need if you have a Lucent PC card to use with an Airport station for wireless internet access. The name rings the bell over here too, but we are probably talking about an older extension predating system 8.6 that was rolled into it eventually like so many others.
