There are two footnotes on the DXCC countries list, February 2002 edition, which clarify this:
T8, KC6 Palau (Formerly Western Caroline Islands) (21) Includes Yap Islands December 31, 1980 and before.
V6 Micronesia (Formerly Eastern Caroline Islands) (38) Includes Yap Islands January 1, 1981 and after.

Micronesia, Federated States of:
In 1979 the Federated States of Micronesia, a UN Trust Territory under US administration, adopted a constitution. In 1986 independence was attained under a Compact of Free Association with the US, which was amended and renewed in 2003.
Palau:
After three decades as part of the UN Trust Territory of the Pacific under US administration, this westernmost cluster of the Caroline Islands opted for independence in 1978 rather than join the Federated States of Micronesia. A Compact of Free Association with the US was approved in 1986, but not ratified until 1993. It entered into force the following year, when the islands gained independence.

When the Caroline Islands were a U.N. Trust Territory, for DXCC purposes they were divided into two countries, Eastern Caroline Islands and Western Caroline Islands, strictly on the basis of their geographical separation.
Then the islands became independent and formed two countries. The Western Caroline Islands became Palau (sometimes referred to as Belau) and the Eatern Caroline Islands became the Federated States of Micronesia.
The Yap Islands had been considered part of the Western Caroline Islands based on their geographical location, but when the Caroline Islands became two distinct political entities the Yap Islands were made part of the Federated States of Micronesia.
