I've had this happen twice. Each time was slightly different. I have dual monitors and the first time it happened, I moved my mouse to the other screen. When the user login box switched to the screen my cursor was on, the password field appeared. When I moved my mouse back to the second monitor, this time, the password field was there.
The second time it happened, both screens were the same with no password field.
