Regarding #4: Notifications of missed alarms (from clock app) seems to has been introduced since OTA-11. You can see it in the screenshot in the middle. It is comming from "Wecker" which is the German wording for "Alarm". It was created after 10 min (the default timeout time) of not responding to an alarm.
