Check /usr/share/notify-osd/icons/hicolor/scalable/status/ and see if the notification-audio-* icons exist. Quite a few apps put their icons in a similar path so it should work.

The icons in the Human theme should not be used in the GNOME default theme, separate GNOME-default-style icons should be placed in the GNOME-default theme, and licensed appropriately. Symptom description: Apparently notify-osd takes over compiz's volume osd, but the problem is that it doesn't work. When I used the volume control buttons on my keyboard, all I see is a empty black box in the top-left corner of the screen.
