I've added some instructions to my DX4WIN Application Note page:
http://dx4win.ad1c.us/appnotes/
Look for the section entitled, "Print Address Labels for Direct QSLs".
If I made a mistake transcribing the procedure, please let me know.

p.s. in the DX4WIN QSL manager database, I do not publish a QSL manager
when it's the same as the home callsign.

I tried DX4WIN 6.04, 7.02 and 7.03 and did not get any "Index out of
range" errors when adding a new entity. Perhaps you have found a path
to a bug that no one else has. The only way to fix it is for you
document every step of a repeatable procedure that stimulates the
problem. It could also be something in your country file that is
corrupted, in which case Paul would need both the procedure and your
country file to duplicate the problem.

I documented a procedure in an application note at http://dx4win.ad1c.us
Because there are several components to adding a new entity, I can't
really see how a different application can be made more user-friendly.
If you follow the steps that I documented, it WILL work.
Don't lose sight of the fact that there IS a bug in DX4WIN that can
cause log data corruption when editing the country file. However, the
procedure I documented works around the bug, again, if you follow all
the steps in order.

When I import LoTW files, I get a few log entries (mostly for US QSO's)
which have the error in the QSO field:
Error:Prefix=291
But South Cook QSO has
Error:Prefix=234
Ideas?
