I tried this but it didn't work, even with the initial "clean" setup. I 
get the exact same issue.

The one thing that I noticed (that should have no effect but I will 
mention it) is that at first start, I get an error about D4W being 
unable to load the LPT drivers. This only happens the first time I start 
it, but since I don't use the LPT port it's not an issue for me.

I will attempt to install a completely fresh copy in another directory 
on my computer and see if that makes a difference.

Dx4win doesn't support Cabrillo
If you are interesting there is an application from sp7dqr, which works for 
some contests (like OK rtty)
Export an adif file from dx4win and convert it to Cabrillo
Sp7dqr software is called adif2cabr and it is here

You can't.  DX4WIN is not a contest logging/scoring program.  I 
typically export ADIF from DX4WIN, then import into WriteLog.  You will 
have to edit the DX4WIN ADIF fields before importing to match what 
WriteLog expects.  You can use N1MM instead of WriteLog.  But you have 
to know what you're doing.
