Thanks to help from John and Scott, KA9FOX, I have updated this
application note describing the configuration process:

The key is the name of the directory into which one installs
(extracts) the callbook data.
Note that QRZ will *not* populate your logbook Name, State, County and
Grid fields. This feature is only available to those using the Radio
Amateur Callbook ("Flying Horse").
