As I have seen, it doesn't work ok for all printers. For example I have a laser printer (HP M1005) which insists to print a4 page... Anyway...
There are 2 places to tell what paper you have.
First in dx4win...
QSO window>QSLs>print QSL labels>Edit
Under paper size box write Custom. Then you will see 2 new boxes named "page width" and "page height" where you should write the dimensions of your QSL card.
Second...
when you are going to print, your printer needs to know what paper you have. If you go to printer's properties you can change the paper also. Maybe you need to change it also. Have in mind that maybe this will change your settings in dx4win edit window above...
