I believe that by "back of the form" you mean the same thing as the "DXCC Record Sheet", which is available separately on the ARRL web site in either text or pdf form. This is (almost) the same as the form prepared by DXbase, except for---you guessed---the fact that *all* three of the forms have a different number of entries. DXbase puts CW first and Phone second. I haven't submitted cards for over 20 years but I'm going to Visalia and would like to have some field checking done, so I would really like the answer to this one. N7WS
