Joe gave me a hand to make the following card design quite a few years ago:
http://www.dxspots.com/CardBack_9Q_ReadMe.htm It's been updated since then and is provided in the DXBase2007 release with the names Cardback_9Q_Black_centered.crd and Cardback_9Q_Black_edge.crd. Which you should select will depend upon how your printer handles QSL card stock (3.5"x5.5")--do you center the card, or do you feed along the edge. To use these go to: Output>User Designed Labels>Select Label Type and choose QSL Cards and QSO's per Label 9. The card used is a design from W4MPY printed on one side and blank on the other. The QSO information is all printed on the blank side by DXBase, as you can see from the above web page. When I am in a QSLing mood I initially print 'labels' on plain paper using the '5160-1qNew.lbl' design to check that I have all the correct information, such as the method (Bureau, Direct, Manager, etc) and if Manager, the Managers call on the label, and that this is consistent for multiple Q's with the same station. Once everything is in order then I print using Cardback_9Q_Black_centered.crd with an HP-1200 laserjet. This has been an excellent system for me. Ron, N5IN
