That's because the example is loading a file without an extension:
javascript: lp.setURL('../REVISION');
if the files are renamed, and the code changed to:
javascript: lp.setURL('../REVISION.html');
it will work fine.
I had done this, but it seems the examples were overwritten with an old
version.
