I don't know if anyone's answered your question already, Matthew, but...
DynAPI.document.all is constructed at runtime when the base DynDocument
is created. When you create a DynLayer using the DynLayer constructor,
and then add it to a DynDocument or other DynLayer, the id of the new
DynLayer should be added to the all[] collection of the associated
DynDocument, to keep track of which DynLayers live in which DynDocument.
I've never taken a good look at inline.js so I'm not sure if it's
working properly with NS6.
