I have had a problem with similar symptoms, in which it turned out that the
rendered size of the form elements was too wide for the table/layer
combination, and that in this instance Netscape would refuse to render any
part of the element rather than clip it.
Alternately I have heard (and keep meaning to get round to testing) that
there may be some issues with using the <input type="filename"> tag in
layers but I thought this was to do with getting/setting its value?
