Guarantee operands in AstOperatorNode are non-null (#1609)
* AstOperatorNode: operands are non-null
This is now a class invariant. Found by cppcheck's
nullPointerRedundantCheck.
* AstOperatorNode: remove dtor and default ctor
They don't enforce the new invariant.
