It comes from the monthly service charge of US$12/month. The US$20 cost of printing the cards can be looked at as a simple promotional expense, especially if it attracts 'serious'/long term users.
Also, while there is no charge for POS transactions, they do charge 1% plus Mastercard International's US$3.85 standard network fee for ATM withdrawals. That isn't all that bad if you're withdrawing relatively large amounts.
