There could be more links in that chain. Delays may be caused by either one of those banks because of various reasons. Main reason is that someone in the bank is someone there not doing their work. Also the receiver might lie about not receiving the wire. Still it's possible to get confirmation from the receivers bank whether they have or have not received your wire. AFAIK, all the SWIFT USD payments go through USA, even if the sender and receiver bank are in the same country. Same applies to other currencies as well. That's what I call bizarre.
