Marc,
CTRL+ALT+L was a shortcut for "Local To Field" refactoring,
which is now merged with "Introduce Field".
In #633 we will have a new shortcut CTRL+ALT+F for "Introduce Field",
and CTRL+ALT+L will be assigned to "Layout Code".

As to defining keymaps: you cannot change keybindings in preinstalled
keymaps. First create your own keymap in  "Tools | Keymaps"
by copying one of the preinstalled ones. Then you can assign keys
to any action you like in this new keymap.
