Can't reproduce even with this use case.
May be it depends on what you've done before 1.
Could you please explain this point ? (1. was on a new document, you previously add comment on other document, etc...)

To reproduce :
1. Add a comment.
2. Click on Reply.
3. Click on Delete.
    -> The comment is deleted.
4. Click on "Add a comment"
    -> The form is not displayed.
