I'm having the same issue.
I've found that this happens when using LDAP (Users in LDAP, groups in SQL),
following the instructions of the 'Nuxeo EP Book'.
When a user adds a comment, appears the message 'Your comment has been
successfully added to topic' and it gets published, but with the text 'Waiting
for approval' next to the title and user name.
If LDAP is disabled everything goes fine
