Rearrange "Manage Subscriptions" screen

Users who have subscribed are in a form table (datainput) with no label, so
they are floating a hundreds pix on the right (see screenshot). They should be
in DIV instead. Please give the DIV (and not the SPAN, i insist) a specific
class.
The whole select/add users and subscriptions should be in a specific DIV with
class too. "search for users" should go on left column with following label
"user selection" (to go with "action" and "notification").
