create BranchedQuestion in one step

a BranchedQuestion element is composed of BranchedQuestionConstainer(
corresponding to ConditionalStepFolder), a BranchedQuestion ( =>
DocumentRouteStep ) and a QuestionBranch( StepFolder ) for each psossible
answer.
for now creation is done in two steps
1. creation of a BranchedQuestionConstainer
2. creation of BranchedQuestion ; when a BranchedQuestion is create a listener
creat a QuestionBranch for each psossible answer.
the goal is to create the BranchedQuestion in one step.
