Though it may be of note that pdftohtml does not support the noframes
option with complex output...

The problem is with the version of pdftohtml (poppler-0.5.4) that
installs from the Red Hat Network.
Have replaced with the latest version (0.10.7), compiling from
source, and all now works.
http://jira.nuxeo.org/browse/NXP-3821 updated accordingly and can be
closed off.
