you may need to update pdftohtml in your system. We had a similar problem (on CentOS 5) that we solved by updating pdftohtml. In our case, pdftohtml was generating the html output (index.html) in the wrong place (the file was not in /tmp/pdf2html_* as expected by Nuxeo, only .png files was there). And Nuxeo was not throwing any error. It was not a Nuxeo bug...
