The one that I have used (including while in Borneo) is International SOS, who does air evac to Singapore or Darwin from Kalimantan (not sure about the Malaysian part, though I assume it's similar). The full travel/medical with evac is between $1500-2000/yr depending upon the usual things (age, sex, country/state of residence, etc.). I don't know about other services but you have to buy for one year at a time. Their web site:
