I meant for the patch to patch the ChangeLog with the new entry.

Yes, that is fine.

I don't think so. It is discussed every so often on the list that
all. Basically i like to minimise the work i and others have to do by
making the patch as easy as possible for me to review, apply and test.
So the little things like using unified diffs, relative to packages,
with ChangeLog and sent as an attachment so it is not mangled by the
mailer all make our life a little easier and speed up the process of
getting patches in.
