Believe it or not, the US taxpayer has helped you out. There is a fairly
decent set of documentation on these data formats at the Medicare EDI web
site, http://www.hcfa.gov/medicare/edi/edi.htm  and several pages in that
neighborhood at the site.

(Although "ASC X12" does not describe a document format. It describes a
public standard used in the creation of documents).
