OOPS - I just noticed in the examples that I didn't have some of the 
qualifiers right:

Product UPC (UCC 12) (UP)            3-58790-00115-9
UPC 1-5-5 (UI)                                3-58790-00115
UPC/EAN Case Code, 2-5-5 (UA)   03-58790-00115

Again, what would these be:

NDC 5-4-2 (N4)
NDC 11 Digit (ND)
