Although I do not think it is immediately obvious from the
documentation, the first segment in any repeatable segment structure is
called the lead segment. Effectively it is used to define the start of
the repeating structure, so it is made mandatory, and it will have a
repetition count of 1 since it would be difficult otherwise to
distinguish between multiple occurrences of the segment in one
structure, and multiple occurrences of the structure which was otherwise
empty (this may not worry you but conversion software can get very
unhappy about it).
Normally the repeating structure is an EDIFACT group, but an entire
message is itself a repeating structure which can occur within an
interchange or functional group, and so the same logic applies to UNH.
