My first impression is, Why do I have to provide contact information and the
answer to a security question if I am answering a survey.  This guy is
selling me something, for certain.  Therefore, I can't trust the outcome of
the so-called survey.

 

I, therefore, F4-ed out, because I never heard of you, and your poll's
appearance is not professional looking.
