Looking at your FDF file

it looks correct.
I would not be surprised if your firmware is truncated or corrupted at boot
time. If I was you, I would start by investigating it is not the case yet.

Driver 5A50AA81-C3AE-4608-A0E3-41A2E69BAF94 was discovered but not loaded!!
upon boot. Are there any dependencies I might be missing?
