I think HTML forms can do most things you need for a web-based questionna=
ire.
Most available software - and even commercial - does nothing but help you=
 code
HTML forms. If you use Java, you also will lose participants who have dis=
abled
this features in their browsers. To get a comfortable data format I can
recommend CGI2SPSS, a Script that saves your questionnaire data in SPSS f=
ormat.
It is available under

ml and
is free for non-commercial use.
