I wonder if anyone aware of a mixture model which combines logistic regression with a survival function. (e,g, Kuk and Chen, 1992. "A mixture model combining logistic regression with proportional hazards regression." Biometrika 79:531-541.) Please point me to a source where I can obtain a routine for estimating such models.
