How about paying the runners-up to review a certain number of diffs in medical articles? You could salt them with ones you know are bad to make sure they are paying attention. Also I am always asking Sue and Erik to put search failover back, at least to PubMed (maybe with a notation to click on "reviews"?) Can you support that?
