It's working fine now. The following procedures fixed it:

1.
Add the following line to /home/httpd/cgi-bin/adv-proxy.cgi
        print FILE "log_fqdn on\n"; 
where the file /var/efw/proxy/squid.conf.tpl is being written.

2.
Change both the DNS server to point to the DNS server that has the reverse
lookup data for the green network.

3.
Setup a forwarder on the DNS server on the green network to forward all
other DNS domains to your ISP's DNS server.

It would be nice if this could be incorporated into EFW community. You know,
have a check box that says: Log FQDNs instead of IP addresses.
