You should check out the FAQ at Squid for more details but the Cache Miss
is literally that it means that what ever page was being requested was NOT
in the cache and therefor a miss. If you have only a few users this is
normal. It is only when there are lots of people that the cache really
starts get high hit rates.

I have TCP access

  *Total Cache Misses:* 118966  *Average Cache Misses:* 4956.91 per hour
*% Cache Misses:* 70.13 %
and TCP transfers

  *Total Cache Misses:* 2 Gb  *Average Cache Misses:* 86.1 Mb per hour  *%
Cache Misses:* 93.11 %

With 150-200 users.
