I have an older Motorola ATA that sits behind my firewall. I have zero inbound
rules currently. I used to use the SIP proxy but it stops logging after a
while. I also used inbound rules below and it worked in all cases. Sometimes
it does stop working in its current config but it picks up after an hour or so
so I blaim that on COMCAST saturation. I do have Static DHCP setup for the ATA
so it gets the same address, you may try that. This will also help on inbound
rules so you dont have to change it. I am using EFW 2.1.2

OK...for those of you keeping track ;) ...I swapped my original "target
host" for another one I had lying around. This one works much better.
  Configured cleanly, etc.

QUESTION:
Has anyone gotten Vonage hardware to work behind the firewall? Using
SIP Proxy, I assume? I found their tech-notes on the ports and it lists
 this:

===================
Ports used by Vonage Adapters

The following ports are needed for OUTGOING Internet communications
from the Vonage device to the Vonage servers.

  DNS: Port 53 UDP

  TFTP: Port 21, 69, 2400 UDP

  HTTP: Port 80 UDP

  NTP: Port 123 UDP

SIP: Port 5061 UDP (used for older Vonage devices provisioned before
 2005)

The following ports are needed for INCOMING and OUTGOING Internet
communications from and to Vonage devices and servers.

RTP (Voice) Traffic: Ports 10000-20000 UDP. When a call is made, a
random port between 10000 and 20000 is used for RTP (Voice) traffic. If
any of these ports are blocked, you may experience one way or no audio.
